Providing a Property Site Sketch and Liability Waiver
Wisconsin Public Service wants to make applying for new electric/natural gas
service an easy and hassle-free experience for you. One of the most critical
pieces of information we need to have, within seven (7) calendar days of applying
for new service is a completed
Property Site Sketch and Liability Waiver form.
It is important to note that work will not proceed on your service installation
until a completed form is received and on file at Wisconsin Public Service.
The Property Site Sketch and Liability Waiver must contain a variety of information
including:
- Signature of the property owner or the authorized representative
- Customer name, service address, city, state, zip code and phone number
- Name of road the building front faces and nearest crossroad
- Building site dimensions in feet
- Footage building is set-back from road and lot lines
- Footprint of structure
- North orientation
- Septic tank with primary and alternate drain fields (private sanitary system)
- Private well and service lateral to the building
- Municipal water and sewer lines on the sketch
- Location of porches, patios and decks
- Location of all underground facilities including, but not limited to: fuel runs,
electric wires, communication wires, waterlines and other
- Future plans for additions, decks or out-buildings
- Preferred electric (E) and natural gas (G) meter locations, including distance
from closest corner. Meter spots on the sketch must be in compliance with all
applicable codes and standards. (Please note: WPS
cannot always guarantee your preferred meter location.)
